COPA - STUDENT ACKNOWLEDGMENT and WAIVER OF COVID-19 RISK

COPA - STUDENT ACKNOWLEDGMENT and WAIVER OF COVID-19 RISK

The Oral Roberts University College of Business Internship Program (hereinafter referred to as “COB Internship Program”) recognizes that students gain invaluable experience in business practice as an intern. As organizations re-open, the risk of COVID-19 infection is still relatively high. Some organizations are allowing students to work onsite, remotely, or a mixture of both.

At this time, the COB Internship Program is offering students the option to accrue internship hours remotely in order to minimize the risk of infection of COVID-19. However, the COB Internship Program will allow its students to work at approved organizations in person with the understanding that there is a risk of infection of COVID-19.

By signing below, you, as a student, acknowledge the following:
• You understand that the COB Internship Program is not requiring you to complete your internship at an organization that expects hours to be accrued in person.
• You are requesting to accrue hours onsite, remotely, or a combination of both at your organization in accordance with the organization’s current operating procedures.
• You understand that it is your sole responsibility to work with the organization to determine the proper safety protocols while continually adhering to CDC, State, and local guidelines.
• Your work will be provided in a manner that minimizes the risk of infection and will be performed in accordance with any local, state, federal, CDC or Oral Roberts University guidelines.
• You will not hold the COB Internship Program, or any other division, agent, individual or entity employed or associated with Oral Roberts University, liable for any damages, harm or infection regarding COVID-19 in relation to your participation in any internship.
• You expressly waive any present or future legal claims you may have against the COB Internship Program, or any other division, agent, individual or entity employed or associated with Oral Roberts University, regarding any COVID-19 claim.
• You agree to adhere to the following ORU Fall 2020 Coronavirus Response Plan, which is herein incorporated by reference—and any update/modifications to it as may be made from time-to-time. By signing below you agree that you have read and consent to the provisions of the ORU Coronavirus Response Plan that can be found at the following address: https://oru.edu/campus-health/.
• You will provide a copy of this form to your Course Instructor.
