ORU QUARANTINE/ISOLATION AGREEMENT
As a result of the national, state and local declarations of emergency surrounding the novel coronavirus and resulting COVID-19 disease (COVID-19), Oral Roberts University (ORU) may be required to impose quarantine/isolation requirements (Isolation) on certain individuals residing on ORU property, including students living in residential housing. Individuals who may be subject to Isolation (Resident) include those who:
a. have received a positive or presumptive positive test for COVID-19 as well as those who are awaiting the results of COVID-19 test;
b. have been in close contact with an individual who tested positive for COVID-19;
c. exhibit symptoms consistent with COVID-19; or
d. are otherwise designated by ORU as appropriate for quarantine requirements.
Depending upon the circumstances, the period of Isolation will likely last at least 14 days and could be longer. The following are the requirements for a Resident subject to Isolation:
1. Resident is to remain in their designed room to the extent possible. Resident may be relocated to different room/floor assignments from time to time by ORU. Under no circumstances is Resident to leave the floor during the period of Isolation with prior authorization from ORU.
2. Resident must cooperate with the efforts of federal and state or local health authorities regarding his/her health condition including by adhering to guidelines for medical care, avoid contact with and exposure to other people, and otherwise prevent the possible spread of COVID-19. This includes providing information regarding people Resident had contact with, places Resident visited or traveled to, and Resident’s medical history.
3. Resident is not permitted to have any visitors at any time on the floor or assigned room.
4. Resident will provide information and undergo such testing, as may be reasonably
necessary, to diagnose or confirm the presence, absence, or extent of infection with COVID-19. Resident agrees to provide ORU with access to all medical records related to COVID-19 including by executing the Authorization Form attached hereto. Resident must cooperate with the instructions of healthcare staff and other authorized personnel during the time you are in quarantine.
5. Should the fire, tornado or other emergency evacuation alarm sound, put on a mask (which will be provided) and proceed down the fire escape. Do not stand near other students.
Failure to strictly adhere to these requirements may result in Resident’s removal from ORU property, which could also impact Resident’s enrollment status. Resident will not be entitled to a refund or credit of any room or board charges. For good and valuable consideration, Resident hereby releases and discharges ORU of and from any and all claims arising out of or related to the subject matter of this Agreement.
