Confirmation and Reporting of COVID Testing

Please fill out this form and upload a copy of your COVID-19 test result. The test must be a PCR or Antigen test. If you haven’t received your test result, please upload the documentation that your test has been administered 5-7 days from your arrival date to campus.

Please be aware that you will not be eligible to move into your room in the residence hall until the test result or documentation that you have taken the test is received by the University. Once your test result is received, please resubmit this form and upload your test result. Thank you!
